• Prescription from your physician written no more than two weeks ago.
• Insurance plan card or workman’s compensation claim number.
• Tank top and shorts to be worn during physical examination.

What to expect:

Expect the first session to take an hour to an hour and a half. An extensive history and evaluation will be performed and treatment and a home program will be initiated. You will work one-on-one with the therapist for 45-50 minutes each session thereafter.
